Community talk. May be wrong. Not medical advice. Not a protocol. Not for human or animal use.
Local — a scalp shampoo for hair talk. Swallowing keto tablets is a different, systemic drug.
Inherited notes record 1% OTC and 2% products at two to three times weekly; inspected users also report once weekly, three to four times weekly or daily. Dryness often determines frequency rather than a proven hair-growth dose response.
Some users report tolerating or preferring daily use; others report dryness, tightness, folliculitis or worse flaking. This is a contradiction, not a recommendation.
The cited current DailyMed label is for tinea versicolor and says one application should be sufficient; inherited notes separately discuss dandruff and seborrheic-dermatitis use. Neither is proof of androgenetic-alopecia benefit.

Timing context & sources
Half-life in the body
Ketoconazole was not detected in plasma during reviewed chronic scalp-shampoo studies, so a systemic half-life was not measurable in that context.
DailyMed reports non-detection in 39 patients using shampoo 4–10 times weekly for six months and 33 using it 2–3 times weekly for 3–26 months.
Non-detection is assay- and exposure-specific; it is not zero absorption and does not apply to whole-body topical exposure or oral tablets.
- DailyMed — Ketoconazole shampoo 2% label (opens in a new tab)Clinical pharmacology: ketoconazole not detected in plasma in 39 patients shampooing 4–10 times/week for six months or 33 patients shampooing 2–3 times/week for 3–26 months; label also lists local adverse reactions.Product-label evidence, assay detection limit not stated in the reviewed excerpt; applies to scalp shampoo, not oral tablets or whole-body exposure.
Felt duration people report
Dryness can appear after the first washes; one same-author report worsened after three to four days and had much less flaking but persistent itch after two months.
Other users report daily tolerance, once-weekly use, folliculitis or no benefit. Concentration, other shampoos, hydrocortisone and minoxidil differ.
Uncontrolled self-reports with different diagnoses, products, co-treatments and frequencies; symptom timing is not systemic PK.
- r/SebDerm — Ketoconazole made my seborrheic dermatitis worse (opens in a new tab)Original and same-author update: 2% shampoo twice daily with hydrocortisone/oral medication; worse flaking and dry tight scalp after 3–4 days; after two months flaking was much reduced while itch persisted.Single diagnosis report with unusually frequent use and multiple concurrent treatments; diet changes and oral medicine discontinuation confound the update.
- r/tressless — Daily ketoconazole shampoo discussion (opens in a new tab)Visible replies span 2% once weekly, 3–4 times weekly and daily use; reports include no dryness, dry hair, hay-like texture, folliculitis, no benefit and concurrent minoxidil/finasteride.Different countries, concentrations, products and co-treatments; no diagnostic confirmation, standardized outcome or product comparison.
Other context in this card
- Piérard-Franchimont et al. — Ketoconazole shampoo and androgenic alopecia (opens in a new tab)Abstract: long-term 2% ketoconazole shampoo compared with unmedicated shampoo with or without 2% minoxidil; hair density, size and anagen proportion improved in the ketoconazole group.Older small comparative study with limited abstract detail; not a modern pivotal AGA trial and not evidence that shampoo replaces established treatments.
- DailyMed — Ketoconazole tablet boxed warning (opens in a new tab)Boxed warning: serious hepatotoxicity including fatal or transplant cases, restricted role when other antifungals are unavailable, and QT-prolonging drug-interaction risks.Oral-tablet labeling is important separation context but does not describe scalp-shampoo systemic exposure.
What people say
- Flake / itch / oil control: The reliable, boring win. forumtrial
- Hair-count adjunct lore: Old small studies and tressless copypasta that 2% left in 3–5 minutes helped density vs unwashed controls. Not a modern Phase 3 AGA drug. trialforum
- Stack “clean scalp so minoxidil works”: Hygiene halo more than a mechanism paper. forum
- What talk does not prove: Replacing finasteride. forum
Doses people talk about
- 1% OTC, 2–3×/week: The default US tressless line (Nizoral A-D). forum
- 2% Rx, 2–3×/week: The “I got the stronger bottle” camp. forum
- Leave-on 3–5 minutes then rinse is the copied ritual. Some leave it longer; dryness rises. forum
- Daily use is disputed: Some 2% users report very dry hair, tight scalp or folliculitis with frequent use; others report daily use without those problems. Community practice more often lands around once to three times weekly. forum
- Framing: Shampoo percent and leave-on minutes — not a mg peptide chart. forum
- Oral keto for hair is not the community move — liver monitoring, drug interactions. trial
How it may feel
- First washes: Dry, squeaky, color-fade on dyed hair. forum
- 2–4 weeks: Flakes usually quieter if that was the problem. forum
- Months: Hair people keep it as a 2–3×/week habit, not a loading blast. forum
- Overwash: Straw hair, broken barrier, more itch — then they back off to 1×/week. forum
Around the dose
- Clock: Evening shower so the hair can dry; morning min foam on a dry scalp. forum
- Do not mix in the minoxidil dropper. Wash, dry, then other topicals. forum
- After workouts if sweat/oil is the trigger. forum
Cycles people discuss
- Stay-on 2–3×/week like a dandruff shampoo, not an 8-week blast. forum
- Drop to 1×/week if the scalp is already dry from RU/min/alcohol. forum
Timing
- Contact time on the scalp is the clock, not plasma half-life. forum
- Not a next-morning DHT blood test story at shampoo doses. forum
More on what it is
- What most people mean: Nizoral in the shower, 2–3 nights a week, stacked under min/fin. forum
- Why hair people use it: Malassezia/dandruff job plus a 1990s–2000s “mild topical anti-androgen / density” lore that never became a finasteride rival. forumtrial
- Not: Not finasteride, not RU-58841, not a peptide, not ketoconazole cream for jock itch as a hair system. forum
- What it is: Imidazole antifungal. 1% OTC (Nizoral A-D in the US) and 2% prescription shampoo. Oral tablets exist and are not the hair protocol. trial
Stacks
- + Minoxidil + finasteride/dutasteride: The big three plus keto as scalp hygiene. forum
- + RU-58841 / pyrilutamide: On different nights if the scalp is angry. forum
- + Ketoconazole cream is a different job (face/tinea). forum
Access talk
- 1% is grocery-store OTC in the US. 2% is often Rx. forum
- Not a research peptide. forum
Storage notes
- Finished shampoo bottle. Room temp, cap shut so it does not dry into glue. forum
- Not reconstituted. forum
Watch for
- Dry, brittle hair / faded dye. forum
- Contact irritation if stacked with alcohol RU the same night. forum
- Not a steroid. forum
- Oral ketoconazole: hepatotoxicity, QT, drug-drug CYP fights — not the shampoo. trial
